Tadanki Technique to prevent Argentinian Flag Sign - Dr Ramakrishna Tadanki

This is a short one minute video in which I show my way of managing an intumescent cataract. Not all intumescent cataracts behave the same way as I have shown in the video. In this case I got a clean mini rhexis but most of the times we have a flap which has to be propagated to make the mini rhexis. The advantage of this technique is that the wide bore of the Phaco handpiece aspirates the loose cortex in a flash, there by preventing the Argentinian flag sign. Steps 1) Stabilise the eye with sins key and then enter the AC with Phaco handpiece in I/A MODE 2) Remove the sinskey and introduce CYSTITOME with the LEFT hand. Preferably a sharp, new one 3) initiate a high vacuum 4) Now puncture the ant capsule. If you have a flap which cannot be propagated with the Phaco hand piece, you can reform the AC before taking out the HP and do the rhexis with Utrata as usual .
