This video highlights 3 points about cataract and premium lens surgery: (Financial disclosure: I have no financial interest in the products discussed in this video). 1 Do patients ever have a monofocal lens inserted, then come to realize that they want full-range of focus to minimize their reliance on glasses and then choose to have their monfocal implant removed and replaced with a multifocal implant? 2 When does it become impossible to remove one implant and have a different lens implant inserted? 3 Why don’t ophthalmologists recommend intraocular lens exchange more often for patients? a. Most patients are happy with their original lens implant b. Most ophthalmologists do not recommend intraocular lens exchange unless it is deemed medically necessary and safe. c. We are already busy enough helping people who want to have cataract or premium lens replacement surgery. If we were to get involved with offering lens exchanges routinely, then there simply would not be enough time available. d. Upgrading from one lens implant to another implant is often considered elective or not medically necessary, the process is complex and patients have to be willing to pay out-of-pocket for elective intraocular lens exchange. e. Most ophthalmologists do not perform elective intraocular lens exchange because the process is very time consuming, the surgery can be complicated and the patients can be more demanding than usual and may have unrealistic expectations. CONCLUSIONS: 1 It is possible to remove and replace lens implants years after they were inserted and insert lens implants today that were not available in past years. 2 When does it become impossible to perform intraocular lens exchange? It can be done pretty much indefinitely if you and your surgeon have a long discussion about the risks and benefits of performing intraocular lens exchange and if you and your surgeon are aligned with regard to realistic expectations about the surgery and the outcomes of repeat surgery. Additionally, both parties have to agree that most of the time, intraocular lens exchange will be considered elective surgery, not covered by insurance and the patient will be fully responsible for the cost of the surgery. 3 Most ophthalmologists have great and valid reasons for recommending against elective intraocular lens exchange surgery. We are highly selective in performing this type of surgery but do offer it for patients that we believe to be appropriate candidates for this type of surgery.
