Simple reposition of a posttraumatic dislocated PMMA PC-IOL, in eye with posterior capsule rupture

This is a patient with congenital cataract operated thirty-five years ago in another clinic and innitially left aphakic. Five years later he came for secondary IOL implantation and we implanted a large PMMA PC-IOL (7 mm/13,5 mm), which fit perfectly on the peripheral capsular remnants. We also performed at that time a large anterior vitrectomy. After 25 years he experienced a blunt trauma in his left eye and he started to have lower vision and diplopya with this eye. At slit lamp, the IOL was significantly temporally dislocated and one haptic was lying on the anterior surface of the iris, hanging through the pupil in the anterior chamber. We repositioned the same large PMMA IOL, in sulcus, by simply pulling in the center and rotating the IOL.
