The video presents the case of a patient with previously uneventful cataract surgery, who experienced, several years after the first surgery, a blunt eye trauma, followed by superior zonular rupture, with dislocation of the IOL & bag complex, which appeared in front of the superior iris. The surgeon tried to reposition the complex by suturing a CTS at sclera in the superior half of the capsular bag, but, eventually, she finished by removal of the whole complex and implantation of a new IOL, which was sutured at sclera. The case was presented in the "Getting it wrong?" symposium during the ESCRS Congress in Copenhagen, Sept. 2025
