The quest is started either as part of the factions main quests or after Bye Bye Love when Joana warns you about bad goings on. Head to Gomorrah and ask any of the generic Omertas where Cachino is. Your inquiry will make him spawn at the bar (that leads to the courtyard) on the casino level. To get him to work with you, you need to pickpocket his ledger or find it in his room. To get his room key, talk to the Gomorrah receptionist and bribe her. His ledger is in the bedroom in the desk in the corner. With his ledger, you can go inform on Cachino to Big Sal (in the Zoara club section of the casino, ask the doorman near the bar to let you up), which is evil since Big Sal and Nero serve the Legion, or you can work with Cachino to take down his bosses who are preparing to commit terrorism against Mr. House and the NCR. Cachino will tell you to deal with Troike and Clanden in order to thwart his superiors. To get Clanden to abandon the Legion's plans, you need to confront him with evidence of his homicidal nature. With 75 lockpick or pickpocketing Clanden's room key, steal some tapes out of his room safe on the suites level. The safe is near his bed actually partially hidden behind a wardrobe. His key also unlocks a maintenance room on the suites level containing a hooker he murdered, which the Omertas used to blackmail Troike. With the tapes, get Clanden to promise to leave Vegas. Alternatively, you can just kill him. To get Troike to quit serving the Omertas, speak with him in lower Brimstone and ask him if he wants you to free him from his contract. With his consent, go speak with Big Sal in the Zoara club. There should be a door near Troike that will transition you to the casino main level with a staircase that leads directly to the Zoara club. Big Sal can be bartered or speech checked to release Troike. Alternatively, you can hack Big Sal's computer or pick his office safe to find evidence or framing Troike. Sal's office is close to him. You should see two doors very close together. The one of the left is his office. Go with Troike and inform him of his freedom or betrayal, and he will agree to cease his gun smuggling operations and assist you in destroying the Omertas weapon supplies. With a speech check, he'll destroy the guns himself. Otherwise, you have to plant his thermite yourself. With the thermite planted, step out of Gomorrah until the objective of destroying guns is complete. Once the Omerta leaders have thoroughly been screwed with, meet with Cachino who will ask in your assistance in ambushing his bosses. The rest of the quest is a cake walk.
