Left carinal sleeve pneumonectomy

A 47 year old lady presented with history of progressive shortness of breath, cough and hoarseness of voice. A CT scan of the neck and chest revealed heterogeneously enhancing mass in the distal trachea, carina and left main bronchus. Because of progressive symptoms, she initially underwent bronchoscopic debulking and biopsy of the specimen revealed adenoid cystic carcinoma. In the absence of metastatic disease on imaging, she was planned for curative surgery and underwent a Left Sleeve Carinal Pneumonectomy via a combined right posterolateral thoracotomy and left anterolateral thoracotomy. The intraoperative picture revealed a 4*2 cm mass at the distal trachea with involvement of left main bronchus. The right main bronchus was free from tumor. Mediastinal lymphadenectomy was also done. The post operative period was uneventful and final histopathology report revealed adenoid cystic carcinoma with negative margins.
