The triceps is supplied by C7 for elbow extension. The wrist flexors are C7, and the finger extensors are also C7. So that will give you wrist flexion and finger extension. The sensory distribution for C7 is the middle finger. So if you see in the examination that sensation of the middle finger is affected, that indicates C7. Therefore, the disc is herniated between the C6 and C7 levels. The reflex of C7 is the triceps reflex. Quizzes 1. A herniated disc between C6 and C7 most commonly compresses which nerve root? A) C5 B) C6 C) C7 D) C8 Answer: C) C7 Explanation: A disc herniation below the C6 vertebra affects the exiting C7 nerve root. 2. Weakness of elbow extension is most likely due to compression of which nerve root? A) C5 B) C6 C) C7 D) T1 Answer: C) C7 Explanation: The triceps, responsible for elbow extension, is innervated primarily by C7. 3. Which reflex is diminished in C7 nerve root compression? A) Biceps reflex B) Brachioradialis reflex C) Triceps reflex D) Patellar reflex Answer: C) Triceps reflex Explanation: The triceps reflex is mediated by C7 and is decreased with C7 radiculopathy. 4. Sensory loss over the middle finger indicates involvement of which nerve root? A) C6 B) C7 C) C8 D) T1 Answer: B) C7 Explanation: The C7 dermatome corresponds to the middle finger. 5. Weakness of wrist flexion is most consistent with injury to which nerve root? A) C5 B) C6 C) C7 D) T1 Answer: C) C7 Explanation: Wrist flexors receive primary innervation from C7. 6. Weakness of finger extension is most consistent with which cervical nerve root injury? A) C5 B) C6 C) C7 D) C8 Answer: C) C7 Explanation: Finger extensors are innervated primarily by C7. 7. Which of the following motor functions is most characteristic of C7 nerve root involvement? A) Shoulder abduction B) Elbow flexion C) Elbow extension D) Finger flexion Answer: C) Elbow extension Explanation: The triceps, controlled by C7, perform elbow extension. 8. During examination, a patient cannot extend the elbow against resistance. Which nerve root is likely affected? A) C5 B) C6 C) C7 D) C8 Answer: C) C7 Explanation: Elbow extension depends on intact C7 innervation. 9. If a patient reports numbness isolated to the middle finger, which disc level is most likely herniated? A) C4–C5 B) C5–C6 C) C6–C7 D) C7–T1 Answer: C) C6–C7 Explanation: A disc herniation at C6–C7 compresses the C7 nerve root, producing sensory loss in the middle finger. 10. Which muscle group is tested to evaluate C7 motor function? A) Deltoid B) Biceps C) Triceps D) Abductor digiti minimi Answer: C) Triceps Explanation: The triceps, innervated by C7, is the key muscle to test for C7 motor function. 11. Which physical exam finding best localizes a lesion to the C7 nerve root? A) Loss of biceps reflex B) Numbness in the thumb C) Weak triceps with absent triceps reflex D) Weak finger flexion Answer: C) Weak triceps with absent triceps reflex Explanation: C7 compression causes motor weakness in the triceps and diminished triceps reflex. 12. Which statement best summarizes the clinical features of C7 radiculopathy? A) Shoulder pain, deltoid weakness, biceps reflex loss B) Thumb numbness, wrist extensor weakness, brachioradialis reflex loss C) Middle finger numbness, triceps weakness, triceps reflex loss D) Little finger numbness, finger flexor weakness, hand intrinsic weakness Answer: C) Middle finger numbness, triceps weakness, triceps reflex loss Explanation: These three hallmark findings localize to C7 involvement from a C6–C7 disc herniation.
