Manual small incision extracapsular cataract surgery performed by Mr Ali Mearza. Typical method now used in developing countries and occasionally used in developed countries when the cataract is particularly advanced. The scleral tunnel is the key to the technique. Vision blue is always helpful if you have it. I typically do a continuous curvilinear capsulorhexis and then add in relieving incisions if necessary as in this case (normally when the cataract is very large/dense). The lens is then dialled into the anterior chamber and a vectis is used for expression. Irrigation / aspiration is then performed and the new lens is inserted into the capsular bag. Often, the wound is sutureless but if in any doubt, sutures are used. The conjunctiva can be sutured or secured with diathermy as is done here.
