40 year old gentleman with recurrent episodes of pain since 2 yr (not investigated during episode of pain), presented with symptoms of cholestatic jaundice since 2 months. USG abdomen and CECT abdomen s/o cystic lesion in the head of pancreas obstructing the lower CBD, rest of pancreas being normal. Nature of cyst is not clear. EUS s/o thin walled cyst with clear fluid content within (no debris) and no solid component within the cyst or its wall obstructing the lower CBD. Complete aspiration of the cyst fluid done (~ 4 ml fluid within the cyst). Cyst fluid is clear and thin, mucin negative, amylase 63000. Final diagnosis - pancreatic pseudocyst causing biliary obstruction. EUS diagnostic as well as therapeutic here.
