Hard lessons I learnt in these cases 1) Most of these patients have shallow AC. Be prepared for a difficult rhexis 2) This case was good with about 3mm space between cuts but if the RK cuts are more closely spaced, one might cut through the RK scars I had a patient who squeezed the eye while doing the initial entry tunnel which pushed the blade through the RK scar. Had to post pone. If there are more cuts, like 12 or 16( I dont know how many they make, never counted before. Today's case had 8 cuts), it is advisable to do a slightly anteriorly placed scleral tunnel while doing the keratome entry. I say anterior because it is difficult to do phaco if one does an unintentional post scleral tunnel . 3 ) Some how I see that these patients are prone for capsular phimosis. I dont know why. Better to avoid a small rhexis or give relaxing cuts at the end 4) Small rhexis is a definite possibility as the AC is tight and one tends to make a small rhexis. Either make relaxing cuts or extend the rhexis where ever possible
