Spreader grafts are used for aesthetic and functional enhancement of the middle nasal vault in rhinoplasty, but correct placement and fixation of spreader grafts is typically more challenging in endonasal procedures. Knotless, “barbed” suture is a relatively new technology that eliminates the need to tie knots while securing tissue. This procedure video illustrates a technique for securing spreader grafts using barbed suture in endonasal rhinoplasty. Briefly, the procedure involves separating the upper lateral cartilages (ULCs) from the septum with a scalpel; securing the spreader grafts temporarily to the dorsal edge of the cartilaginous septum with a hollow-bore needle; then securing them permanently with a running horizontal mattress technique using a barbed suture (3-0 V-Loc). The spreader grafts and dorsal septum can then be carved further to ensure the appropriate dorsal contour. Barbed suture (3-0 Stratafix Symmetric PDS) is used to reattach the ULCs to the spreader graft–septal complex in a running horizontal mattress fashion. See https://ja.ma/2To2cFe for complete details.
