Midface and mandibular reconstruction after ablative surgery remains a challenging problem. Complex maxillofacial three-dimensional reconstruction is crucial with regards to occlusion, function and aesthetic appearance. In-house 3D laboratory became routine tool in daily practice. Quality of life assessment is important to achieve improvement and excellence in the treatment of maxillofacial malignancies. Simultaneous implant placement and free flap surgery for jaw reconstruction is a fascinating and complex area within maxillofacial surgery. The approach is often used to restore both form and function for patients who have undergone tumor excision, trauma, or other pathological conditions affecting the mandible or maxilla. This technique involves placing dental implants during the same surgical procedure as the flap transfer. This can enhance healing by setting dental implants in a newly vascularized site, potentially improving osseointegration. The timing of implant placement is critical. During the initial phase, surgeons must ensure that the implant can be stable enough to withstand functional loads and integrate properly with the surrounding bone. Performing both procedures at once decreases the total time a patient spends in surgery and under anesthesia. Surgical techniques followed by classy medical drawings describe every step of this unique protocol in detail. Immediate placement of implants can lead to better esthetic outcomes and quicker functional rehabilitation. Patients may avoid multiple surgical interventions that would have been necessary if implants were placed in a staged manner. An interdisciplinary approach is essential, involving maxillofacial surgeons, prosthodontists, and sometimes oncologists or other specialists. From an oncological point of view, simultaneous implant placement is considered a safe and promising approach. Our studies have shown excellent success rates and osseointegration rate for immediate implant placement in patients receiving vascularized bony free flap reconstructions. The placement of immediate dental implants at the time of primary reconstruction of the mandible and maxilla is a safe procedure and facilitates timely dental rehabilitation.
