During the Jul. 8 HAI@Work webinar, HAI President and CEO James Viola and panelists Tarek Loutfy, senior operations and flight safety manager at GE Aviation; Duke Puharich, director of safety at Siller Helicopters; and Robert Lakind, aviation attorney at Szaferman, Lakind, Blumstein, & Blader, discussed causes and symptoms of fatigue as well as countermeasures viewers and their employers can take to mitigate the risk of fatigue contributing to an accident or incident. Speakers stressed to viewers that the most important defense against accidents is their voice, urging them to speak up when they see coworkers behaving abnormally or when they are unhappy with their company’s working conditions and safety culture. Additionally, the group discussed reporting programs that could protect employees and their employers when inadvertent mistakes occur, the process of an FAA investigation, and how to properly write a response to an FAA Letter of Investigation.
