This video features Dr. Vasudevan K.R., who leads the liver transplant and HPB surgery team at Jaypee Hospital, Noida. He has been practising liver and HPB surgery for the last 15 years. He explains two sources of liver donation which include: Living donation: A relative can donate a portion of the liver, usually two-thirds. Deceased donation: A patient that is brain dead and is kept alive with the help of machines in the hospital. In such a situation, the family wishes to donate many organs, including the liver. Furthermore, he mentions how the law allows only a relative to donate, showcasing pure emotional attachment and love for the patient. So it is a legal requirement in India that the donor is related to the patient. However, from a medical point of view, a non-blood relative can also donate the liver, and the law accepts it if the intention behind the donation is pure love for the recipients and it happens rarely. The process to obtain approval is also very long and occurs if the patient and donor have known each other for decades and are of similar financial standing. Additionally, around 40-60% of a healthy donor’s liver is removed and transplanted into the sick patient, and it is a relatively safe procedure. Moreover, the additional safety margin in liver donation, especially in living related or partial living donors, is that the liver grows back to nearly its pre-donation size and volume in 2 months. In the case of a deceased donor/brain-dead donor, the quality of liver function needs to be evaluated by doing a blood test and some imaging to make sure the liver isn’t damaged during an accident, which might have caused the brain death. However, in living donation, two things are evaluated: the quality and the amount of the liver one can get and if the donor can withstand the operation. For the same, multiple blood tests, C.T. scans and MRIs are conducted to estimate the quality of the liver and its functions, and a whole body evaluation is also conducted.
