By the time a woman comes in to us requesting a breast reduction, they've been thinking about it, they're bothered by their back, neck and shoulder pain associated with large breasts, and they want this operation. The nice thing from our perspective is we know with a high degree of certainty who can solve those problems with a breast reduction and have a very happy patient. Each patient is different, and so there are different approaches to each patient. The simplest form of breast reduction would be to liposuction the breast. Now, sometimes we can do that and sometimes we can't. Sometimes the breast tissue itself is too firm and it's more difficult to break up, but sometimes we can achieve the results we want simply by taking out some of the internal volume and not doing anything with the outside skin. Liposuction, however, is probably one of the least likely techniques that we're going to employ because most women require more extensive reduction and not just taking out the inside tissue, but retailoring the skin envelope. As we retailor the skin envelope, we're going to make an incision around the areola, which is the darker skin surrounding the nipple, typically we'll make one down the center of the breast and oftentimes in the crease of the breast. Through these incisions, we'll go in and not only remove the extra tissue, but retailor the skin envelope so that it adapts well to the underlying breast shape that we have created.
