In this video I describe how to use a micropuncture kit to place a 14g angiocatheter. When large-bore IV access is needed for fluid/blood product transfusion, this technique can be performed with an exiting IV or through a fresh stick using a Seldinger technique. The .018" micropunture wire will fit through a 22g IV but, if a fresh stick is going to used, I will frequently use a 20g angiocath to gain initial vascular access through which I will perform this rewiring technique. Key points: the wire and dilator are are extremely flimsy and will kink easily; pinching the dilator and catheter together as you advanced will provide more stiffness and prevent kinking; during insertion, apply counter traction in the opposite direction to prevent folding of the skin and soft tissues; when ever possible, perform under sterile condition; practice this technique through simulation to get a feel of the technique before applying it in clinical practice.
