Advance Care Planning with People with Dementia

An Advanced Care Plan enables health and care professionals working alongside people living with dementia to spend time talking with them (and their carers where appropriate) about their future care. This opportunity to discuss the future will be limited by the fact that at some point in their dementia journey, people with dementia won't meaningfully be able to contribute to that process, so it is important to have more - and earlier - conversations about their future wishes. This short video will help support you to feel more confident in having these conversations. Please share this video with health and care professionals staff but please don’t share it on social media channels – we have consent from participants for it to be shared publicly but it is primarily intended to be used for the purposes of staff awareness raising and training.
