RetinaRounds™ 107: Recurrent Retinal Detachment

Continuing our week of office-based surgery cases, guest surgeon Dr. Omar Shakir demonstrates repair of a recurrent retinal detachment using 27 gauge instrumentation.  This pseudophakic patient first underwent primary vitrectomy with gas implantation for a superior retinal detachment.  At postop month 4 the superior retina remained attached but the patient developed a new inferior retinal detachment.  The break could not be visualized with scleral depressed exam in the clinic and attempts were made to barricade the subretinal fluid with laser although the fluid broke through the laser and is now encroaching on the macula. At the end of the case we discuss surgical options for cases like this and choices for tamponade agent. Thank you Dr. Shakir for sharing this case!
