DALK or Deep Anterior Lamellar Keratoplasty is a type of partial thickness Cornea Transplant, where the anterior or front layer of the Cornea are replaced while the last layer of Host Cornea is Healthy and is preserved while doing the Cornea Transplant. Advantage of Lamellar or Partial Thickness Keratoplasty is reduced rejection rates as compared to full thickness transplant. Though technically challenging it is very helpful in give long lasting results to the patients due to its low rejection rate. DALK is done for a no. of condition especially where the last layer or the Descemet's Membrane is healthy. Few Common conditions are Corneal Scar post Trauma or Infection, Keratoconus and Corneal Dystrophy. This is a case of Granular Dystrophy where one transplant has already been done about 10 years back. Due to the recurrent nature of disease the cornea s again opacified with much deeper lesions.
