Compartment syndrome is a condition in which increased pressure from any source within a closed space compromises the circulation to the tissue contained within this space. Without urgent decompression, tissue ischemia, necrosis and irreversible damage will occur within 6-8 hours. Compartment syndrome usually occurs in the anterior compartment of the lower leg. Symptoms of Compartment syndrome include: •Worsening pain •Tense compartment •Pain with passive stretch •Sensory deficit •Motor weakness is late •Pulses usually not affected. •Pain is more than what would be expected from the injury or surgery. •Pressure measurement Compartment pressure greater than 30 mmHg or within 30 mmHg of the diastolic pressure, usually requires an immediate fasciotomy. This is how fasciotomy is done for the lower leg •Lateral leg incision is made halfway between the tibia and the fibula for release of the anterior and lateral compartments. •Medial leg incision is made 2 cm posterior to the tibia. Thigh fasciotomy •Spittling the tensor fascia lata is required to release pressure on the anterior thigh compartment. (case pictures of tensor fascia lata opened and decompression of the anterior compartment) . Foot fasciotomy •Incisions made to release pressure to the central, lateral and medial compartments of the foot. Hand fasciotomy •Dorsal incision of the hand inline with the 2nd and 4th metacarpals. Forearm fasciotomy •Volar: incision to release the pressure to the volar compartments of the forearm. •Dorsal: incision to release pressure to the dorsal compartments of the forearm.
