Darrach and Sauvé-Kapandji (SK) procedures are two surgical techniques used to treat distal radioulnar joint (DRUJ) dysfunction, often caused by arthritis, trauma, or instability. While both aim to relieve pain and restore forearm rotation, they differ in approach and long-term outcomes. 🛠️ Darrach Procedure What it is: Resection (removal) of the distal ulna just proximal to the sigmoid notch. Goal: Eliminate painful contact between the radius and ulna. Best for: Older, low-demand patients. Pros: Simple and effective Good pain relief Cons: Risk of ulnar stump instability May cause ulnar carpal shift and weakness in grip 🔧 Sauvé-Kapandji Procedure What it is: Fusion of the DRUJ combined with creation of a pseudarthrosis (false joint) in the ulna just proximal to the fusion. Goal: Maintain forearm rotation while stabilizing the joint. Best for: Younger, active patients or those needing better grip strength. Pros: Preserves pronation/supination Better grip strength and return to work rates Cons: Technically more complex Higher risk of nonunion or hardware complications 📊 Comparative Insights Both procedures improve pain, wrist range of motion, and forearm rotation. SK tends to outperform Darrach in grip strength and functional recovery, but may have a slightly higher reoperation rate2. #Darrach #Sauvé #Kapandji #SurgicalProcedures
