Key Concepts: Definition of Cold Abscess Abscess without signs of acute inflammation (pain, redness, heat). Etiology Commonly due to tuberculosis, fungal infections, or non-pyogenic bacteria. Pathophysiology Slow necrosis and liquefaction of tissue. Common Sites Cervical lymph nodes, spine (Pott’s disease), chest wall, psoas muscle. Clinical Features Painless swelling, fluctuation, no redness or warmth. Differential Diagnosis Compare with acute abscess, lymphadenopathy, cysts, tumors. Diagnosis FNAC/biopsy, USG, CT/MRI, AFB staining, TB PCR, culture. Treatment Options Anti-tubercular therapy (ATT) in tubercular cold abscess. Aspiration or surgical drainage if large or compressive. Antibiotics (based on etiology). Complications Sinus formation, recurrence, secondary infection.
