Lumbar transforaminal epidural steroid injection is an outpatient treatment option that reduces the swelling of spinal nerves by injecting a steroid-anesthetic medication. It is also done for lower back pain, sciatica, pinched nerve, sciatic nerve pain, and herniated disc. The medical term foramen refers to the opening between the vertebrae and at the side of the spine where a nerve root exits. The word epidural means "around the spinal cord.” Lumbar epidural steroid injection is usually performed for chronic pain, mostly radiating into the lower extremities. The chronic lumbar pain is also known as sciatica or radiculopathy may or may not be associated with numbness, tingling as well as weakness of the lower extremities. The procedure is performed either for therapeutic reason or diagnostic purpose. The diagnostic purpose injection is performed to evaluate the cause of the pain. We know that there are 5 lumbar nerves which are passing into the lower extremities. The pain may come from any one or more of the 5 lumbar nerves. The reason for which diagnostic injection is performed is to find out the level of the pinched nerve, either by disc herniation or bulge disc and to perform the injection at that level so that the pain is relieved and the cause of the pain is possibly treated with or without surgical treatment. If the patient is not a candidate for surgery or any other treatment and not responding to oral pain medications and any other treatments, then the therapeutic injection is performed. This therapeutic injection is performed multiple times and may be repeated every 3 to 6 months as per the need. How is Lumbar Transforaminal Epidural Steroid Injection Done? The patient is brought into the operating room and will be asked to lie in prone position. A pillow is kept underneath the patient's stomach. An x-ray is used to view lumbar vertebrae. Now the diagnostic injection is performed at the selected nerve, it may be any of the L1, L2, L3, L4, or L5 lumbar nerve. The diagnostic injection level depends on the findings of clinical examination, CAT scan, MRI studies, and EMG examinations. If the patient is scheduled for diagnostic injection, then 1 or 2 level injections are performed the same time. If the patient is scheduled for therapeutic injection, then up to 3 levels of injections are performed the same time. Once the patient is taken to the operating room, put on the bed in prone position, the x-ray is used to identify the level of the foramina where the injection is to be done. The local anesthesia is used to anesthetize the injection site. Once the foramen is viewed using the x-ray, the needle entry point is marked with marking pen. A 1 to 2 cc 1% lidocaine local anesthesia used for skin and subcutaneous tissue. A 25 gauge needle is used to inject the medication. The needle is very carefully introduced into the foramina. Cortisone injection used is either Kenalog or Depo-Medrol. After the lumbar epidural steroid injection is done, needle is removed. The patient is sent to the recovery room and observed for any further complication. Patient is discharged home if there is no complication. What Are The Side Effects Of Epidural Steroid Injections?
