Extrapleural Pneumonectomy Does Not Improve Overall Survival in People with MPM

https://www.understandmeso.com/post/e... Electing surgery in the treatment of malignant pleural mesothelioma is a controversial topic. Researchers conducted a study to determine the efficacy of extrapleural pneumonectomy (surgery involving the complete removal of a pleural mesothelioma patient's affected lung, the pleural lining of the heart and chest, parts or all of the diaphragm, and any cancer visible to the naked eye) in the treatment of malignant pleural mesothelioma because chemotherapy and radiation are relatively ineffective. Extrapleural pneumonectomy is the standard in the treatment of malignant pleural mesothelioma because 1): it allows complete removal of the tumor and 2): can be associated with long-term survival. The researchers conducted their study at multiple institutions and involved patients with untreated malignant pleural mesothelioma. From September 1985 to June 1988, 83 eligible patients (64 male, 19 female) were entered into the study. The mean age for all patients was 59.7 years. Only 20 of the 83 patients underwent extrapleural pneumonectomy. Three of these 20 patients died postoperatively. The patients who underwent surgery had longer recurrence-free periods (time in which the tumors did not reform) but there was no difference in overall survival between those who had surgery and those who did not. The researchers concluded that (1) only a small proportion of all patients with malignant pleural mesothelioma are candidates for extrapleural pneumonectomy, (2) extrapleural pneumonectomy carries a significant operative mortality and does not seem to improve overall survival compared with more conservative forms of treatment, (3) extrapleural pneumonectomy alters the patterns of relapse, and (4) factors previously thought to have an impact on survival in other series did not affect the outcome in this trial.
