The video describes a high-velocity low amplitude (HVLA) thrust technique to the costotransverse of the mid-lower (T4-9) thoracic spine. KEYS TO SUCCESS: patient position: seated with arms folded over the chest; the patient self selects their preferred arm folding orientation (i.e. Left over right or vice versa); patient NEEDS to be relaxed and their head is resting on the shoulder of the clinician patient is initially side-bent to the contralateral side in order to "open" up target rib and to hook the pisiform process under the target costotransverse joint (NOTE: the hands of the short lever applicator SHOULD BE pointed inferior lateral following the directionality of the ribs) **with the contralateral side bending the SPINE IS LATERALLY FLEXED but no weight shifting of the patient need occur patient is LATERALLY FLEXED TOWARD and ROTATED AWAY from the side of the thrust the table height needs to be high enough that the clinician can gut under the intended rib without going into spinal extension; think SHOT PUTTING with the thrust (athletic stance is needed)
