Alar Base Modification & Asian Rhinoplasty with Dr. Kian

I’m pleased to share with you today’s 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗖𝗮𝘀𝗲 𝗛𝗶𝗴𝗵𝗹𝗶𝗴𝗵𝘁. Sue and I had the opportunity to meet when I was doing a training in Miami, Florida. And she said, “Look, I want to improve the tip of my nose and my nostrils. Can I do this simply by reducing my nostril width?" Nostril-shaping or refinement is a common request during a consultation for primary and revision rhinoplasty, and is part of every single rhinoplasty procedure, whether it’s maintaining the natural contour of the nostril, bringing the nostril in, or changing the nostril shape. We call this an alarplasty, or an Alar Base Modification. We discussed her goals, which were focused on maintaining her Asian heritage and to look like herself, she was really bothered when she would smile, her nostrils would flare just a bit much more than she cared for. She did ask me about the tip of her nose, and I actually thought the tip of her nose really fit her face, and that she did not need a tipplasty. But we did agree to an alar base modification. So she flew all the way out to Los Angeles. We did it under local anesthesia, it took less than an hour. Recovery was super easy. She had no pain and as you can see, her results are pretty remarkable. I only removed about 5mm of extra tissue from her nostrils, but we did it in a tasteful and artistic fashion to make sure that her nostrils still have a normal rounded appearance and that she looks like herself.
