Gralnek, Cesare Hassan, Jean-Marc Dumonceau This Guideline issues recommendations for the management of anticoagulants, antiplatelet agents, and direct oral anticoagulants (DOACs) in patients undergoing elective endoscopic procedures. Regarding anticoagulants and antiplatelet agents, recommendations are outlined in two algorithms to replace the older approaches. With respect to DOACs, it is recommended that: (1) for low risk endoscopic procedures, the morning dose of DOAC should be omitted on the day of the procedure; and (2) for high risk endoscopic procedures the last dose of DOAC taken at least 48 hours before the procedure, except for patients on dabigatran who present an altered or rapidly deteriorating renal function – they may require more specific care as detailed in this Guideline.
