This 3yo presented with a congenital posterior polar cataract with this video showing detailed technique and special considerations. Key points is the performance of a posterior capsulorhexis (PCCC) and use posterior optic button hole (POBH) to place IOL optic thru while haptic remain in bag. This also obviates the need for vitrectomy which is a major benefit, and there is no scaffold for lens epithelial cells to form PCO over the visual axis as the acrylic optic and edge create a barrier. Use of trypan blue, Healon5 viscoelastic, DK capsulorhexis forceps with markings, Chang cannula, polymer IA tip, Singer sweeper capsule polishing and a 3 piece hydrophobic IOL are shown. With a 2.2mm limbal incision, a suture is not needed and avoids postop sutures issues. IOL power was chosen to leave the patient hyperopic as over time myopic shift will occur. Postop refractive error correction and ambylopia management is critical for visual rehabilitation.
