Generalized zonular weakness secondary to Pseudoexfoliation is a challenge for us especially while dealing with a dense nucleus. In this video i am demonstrating the planning of each of the steps and execution of the same to achieve a successful outcome.Appropriately sized rhexis , good hydrodissection and early introduction of CTR to stabilize the bag are critical steps. Careful division and emulsification of the nucleus is critical and can be achieved . To ensure long term stability of the lens i am using the IOL trap technique where in the CTR placed in the sulcus which provides equatorial stretch and support, the multi piece hydrophobic IOL is placed in the Sulcus with its optic being captured in the CCC. This has worked well for me such eyes with a generalized zonulopathy due to pseudoexfoliation. I have follow up of more than 20 eyes which have been operated 4- 5 years back and still continue to do well.
