These methods target areas of the body innervated by central nervous system pathways, ensuring that any reaction is a reliable indicator of brain activity rather than just a reflex from the spinal cord. Common techniques include: Sternal rub – Applying firm pressure with the knuckles to the center of the chest (sternum). This creates a deep, uncomfortable sensation that is difficult to ignore if the patient has any awareness. Trapezius squeeze – Pinching or squeezing the trapezius muscle at the top of the shoulder. This produces a strong painful stimulus that typically provokes movement or a facial grimace if the patient is responsive. Supraorbital pressure – Applying pressure with a thumb just above the eye, in the supraorbital notch where a nerve runs. This is highly uncomfortable and useful when other methods are not producing a response. These techniques are performed only after simpler methods—such as calling the patient’s name or gently shaking them—fail to get a response.
