An adult female presented with a 3 cm complex cystic mass at the angle of the mandible. Ultrasound imaging revealed a well-defined cyst showing a spongiform pattern with fine internal reticular septae. The contents appeared thick, viscous, and colloid-like, demonstrating slow movement with probe pressure, indicating highly viscous internal material. Color Doppler showed poor vascularity. Given the slow growth over one year and the ultrasound characteristics, the most likely diagnosis is a second branchial cleft cyst containing thick proteinaceous debris or cholesterol crystals. This is typical of long-standing branchial cleft remnants. Differential diagnoses include a cystic metastatic lymph node (particularly from papillary thyroid carcinoma), although the presence of viscous colloid-like matter makes metastasis less likely. Other possibilities like lymphangioma and dermoid cyst are considered less likely due to the nature of internal contents and location. Fine-needle aspiration cytology (FNAC) can help confirm the diagnosis if needed.
