There are four distinct phases of learning cataract surgery regarding reducing the incidence of complications; 1. Phase 1 is up to 87 cases. The complication rate is 3.18% 2. Phase 2 is 88 to 189. The complication rate is 1.68%. 3. Phase 3 is from 190 to 534 cases. The complication rate is 0.79%. 4. Phase 4 is cases 535 to 711. The complication rate is 0.18%. The experience speed stabilised after approximately 123 cases. Trainee surgeons required 535 cases to achieve complication rates comparable to experienced surgeons having a complication rate of 0.2%. It is very interesting to note that 41% of all complications occurred with surgeons who had completed less than 535 cases of cataract surgery. The complication in this series in the first 50 cataract surgeries was 3.1%. The results from this study are very helpful to guide both trainee cataract surgeons and their supervisors. It helps with choosing the cases that the trainee should attempt. It’s important to classify cases prior to cataract surgery into categories of routine, complex and very complex. It confirms that complex cases should not be attempted within the first 87 cases and very complex cases not until at least 190 cases have been completed by the training surgeon.
