Prevention and Treatment of Soft Tissue Failures in Complex Esthetic Cases Presented at AAID's 2015 Educational Conference Marius Steigmann, DDS Adjunct Clinical Associate Professor, University of Michigan Department of Periodontics; Adjunct Assistant Professor of Oral and Maxillofacial Surgery, Boston University; Adjunct Assistant Professor, University of Pennsylvania Department of Endodontics -Founder and Scientific Chair, “Update Implantologie Heidelberg,” 2002-2012 -Private practice, Neckargemünd, Germany Clinically, it is a tremendous challenge to maintain or create natural gingival esthetics following implant placement. Stability of the soft tissue during implant surgery is of major importance for the implant site and adjacent teeth, especially when grafting procedures are needed. In complex cases in the esthetic zone when major soft tissue manipulation is needed, new ways to perform incision, flap design, and suturing become necessary. Learning from our clinical experience in soft tissue manipulation, we can change soft tissue surgery. This lecture will define predictable soft tissue manipulation for implants according to soft tissue biotype. Adjusting therapy to biotype for soft tissue thickness is changing the way we are approaching implants. With the increasing number of implants placed in the esthetic zone, immediate or delayed, we face soft tissue complications like recession of the soft tissue and papilla loss. In cases of delayed implant placement after healing, soft tissue reconstruction of midfacial and papillae in single and multiple implants is more difficult to achieve. There are no predictable methods in the literature regarding how to increase the soft tissue height for the papillae or correct recessions around implants. Hence, treatment options for these cases are needed more and more. A surgical/prosthetic algorithm is necessary. To achieve all these results from a soft tissue prospective, complex planning of surgery, grafting and prosthetic is absolutely necessary. Learning Objectives: At completion of this presentation, participants should be able to: 1. Understand the importance of the emergence profile in the esthetic zone and the specific requirements for natural soft tissue 2. Understand the surgical possibilities of correction of this transition zone between implant and crown 3. Identify the manipulation technique for soft tissue in complex esthetic cases 4. Familiarize treatment sequence associated with multiple implant surgical and prosthetic techniques 5. Build treatment plans based on individual circumstances 6. Utilize modern prosthetic technology for reconstruction of soft tissue for single and multiple implants 7. Plan the surgical position of the implant prosthetically and esthetically
