A recent study aimed to answer this by examining how high doses of Adderall XR (80mg daily) affected patients with ADHD and cannabis use disorder. Over a period of three months, researchers compared amphetamine to a placebo to see whether the stimulant would improve both ADHD symptoms and help reduce cannabis use. The results? Despite the high dose, the amphetamine didn’t improve ADHD symptoms or help patients quit cannabis. This study mirrors previous findings from trials on methylphenidate in teens and atomoxetine, which also showed no improvement in ADHD symptoms or cannabis use. Though the data isn’t definitive, it suggests that ADHD medications may not work as well when marijuana is involved. The real question is: how much marijuana does it take to block the benefits of a stimulant? We don’t know yet, but the evidence we do have points to a problem. Marijuana has been shown to interfere with executive functioning and can even cause ADHD-like symptoms. For clinicians, whether to prescribe ADHD meds to a patient using marijuana will depend on individual circumstances. But it's important to inform your patients that their treatment might not be effective while they are still using cannabis. This might come as a surprise, as many patients believe that marijuana helps their ADHD.
