Some cases of atrial fibrillation are particularly persistent or resistant to treatment. In these situations, one approach may not be enough and we may need to consider two approaches. We call this hybrid or convergent atrial fibrillation ablation. This is a combined effort between the cardiologists who do the catheter based ablation and surgery, where we do a keyhole approach to get the outside of the irregular focus. A surgeon isolates the pulmonary veins and treats the surface of the heart from the outside, and then electrophysiology, cardiology will go from within and ablate the remaining parts and join the dots, so to speak. Now, this is a dual approach and creates a much more complete and durable effect into the medium and the long term. And of course, surgically we can also close the left atrial appendage. In fact, we can actually close the left atrial appendage through the groin also, but this is at the moment, a little less reliable. Now, convergent procedures are considered in more advanced AF or when both rhythm control and stroke prevention are a priority. It's not for everyone, but when chosen carefully, the results have been shown to be excellent in randomized trials.
