HARD CATARACT PHACO :: SIMPLIFIED WITH TERMINATOR THE WEDGE TOOL:T CHOP: ASCRS 2019: Rajendra Prasad

We describe an efficient new device “Terminator” the wedge tool, a modified classical simple machine to crack and break mature hard cataracts. Wedges have been successfully used in rock excavation system since the ancient time to crack and break solid hard rocks. It functions by converting a force applied to its blunt end (base) into forces perpendicular (normal) to its inclined surfaces. Wedges when pushed or driven into a solid hard substance, generates inside out dispersive mechanical force which overcomes the tensile resistance of solid materials and causes them to crack and split in tension. Wedge amplifies the magnitude of driving force, which means it applies more force to the object (output force) than the force applied to the wedge tool (input force). This amplification, or mechanical advantage is the ratio of the input force to output force which is always more then 1. Wedges, can crack and separate thick and hard materials, such as solid hard rock and hard metals with much less force and with more precision, than the compressive crushing force which is the application of the same force over a wider area of the material to be cracked and separated. Consonant to the principle of wedge tool and Griffith’s theory of brittle fracture, we designed a new device named terminator to induce inside out dispersive cracking force to crack and break these mature hard cataracts. The tip of the terminator is incorporated with triangular wedge design in which pointed apex of wedge is the inner edge of the tip where portable inclined plane join to form an specific angle while wide and flat base of wedge is at the posterior surface of the tip. Simply drawing the wedge inner edge of the terminator within the equator of nucleus generate full thickness nuclear crack which automatically traverses through the entire nucleus breaking it into two complete segments. Terminator the wedge device makes surgical maneuvers much easier and facilitates the overall procedure with higher level of efficiency and safety.
