In this video, we're going to discuss a detailed approach to burn injury management. There are many aspects we have to focus on when managing a patient with burns as they can have multi-system involvement. #burn The initial assessment and management of severely burned patients is similar to the approach of a major trauma patient where they will follow the ATLS protocols. The treatment protocols will vary according to the degree and severity of burns. In this video, we will guide you on how to treat a burn (severe) as they have high mortality and morbidity and by being educated we can minimise the mortality and morbidity. It is essential to ensure your safety before approaching any individual. And once approached, go through the primary and secondary survey to assess the patient in an extensive manner. Some patients are eligible to be admitted to a burns unit and that depends on specific requirements. When administering fluids it's important to know the types of fluids and the rates of administration. UOP is very helpful in the monitoring however, the actual fluid rate that a patient requires depends on the severity of injury. Analgesia is required to reduce early metabolic stress and prevent post traumatic stress disorder. Feeding should start within 6 hours of the injury and albumin infusion and IV potassium supplementation should also be done to maintain homeostasis. Sterile precautions must be rigorous when treating these patients as they're immunocompromised & susceptible to infections. More complex burns may require excision and skin grafting and escharotomy is offered in suspense of compartment syndrome. Tetanus prophylaxis should also be done as burns are considered to be tetanus-prone. Lastly, physiotherapy should be started on the first day itself. Psychological support should also be considered as burns can be a traumatic experience. In a summary, when managing a patient with burns you have to focus on; 1. Fluids resuscitation 2.
