Speaker: Dr. Rathan Shetty Topic: Osteoradionecrosis In this video, Dr. Rathan talks about: a. An overview of osteoradionecrosis Osteoradionecrosis (ORN) is a serious but uncommon complication following radiotherapy, characterized by the exposure and failure to heal of irradiated bone. Modern radiotherapy techniques have reduced its incidence compared to older methods. b. Risk factors and clinical presentation of osteoradionecrosis in head and neck cancer Key risk factors include tumor location near the mandible, high radiation doses, poor oral hygiene, and concurrent chemoradiation. Clinical presentation varies from asymptomatic exposed bone to pain, discharge, non-healing fistulas, and pathological fractures. c. Diagnosis and staging of osteoradionecrosis Diagnosis involves clinical examination, imaging (CT/MRI), and exclusion of recurrence, with biopsy taken cautiously. Staging systems, modified, categorize ORN based on clinical and radiological findings, as well as response to hyperbaric oxygen therapy. d. Prevention strategies for osteoradionecrosis Preventive measures include thorough pre-radiotherapy dental checkups, oral hygiene maintenance, smoking cessation, nutritional support, and careful radiotherapy planning to minimize dose to the mandible. Avoiding dental extractions post-radiotherapy and managing dentures are also crucial. e. Medical management of osteoradionecrosis Medical management for early or less severe ORN involves maintaining oral hygiene, antibiotics for infection, and the pentoxifylline, tocopherol, and clodronate (pentac) regimen, which has shown promising results in promoting healing. Hyperbaric oxygen therapy is not recommended as a prophylactic measure and has limited evidence for treatment, except possibly in very early stages. f. Surgical management of osteoradionecrosis Surgical intervention is indicated for ORN refractory to medical management, involving debridement and sequestrectomy for partial thickness cases, and segmental resection with reconstruction (often using free flaps) for full-thickness ORN to achieve bleeding margins and restore bone continuity. Obturators may be considered for patients unfit for microvascular surgery.
