Laparoscopic peritoneal lavage (LPL), while less invasive than laparoscopic sigmoidectomy, was associated with higher overall morbidity (33% vs. 18%) in prospective, multicenter trial involving 66 patients with pelvic abscess and purulent peritonitis caused by acute diverticulitis. In addition, LPL patients were more likely to return to operating room and fail to achieve source control of the peritoneal infection. Dr. Dario Tartaglia says the unfortunate conclusion is that LPL should be used only in patients able to tolerate a persistent or recurrent status of sepsis and multiple operations to control ongoing infection.
