This approach is similar to peeling the onion to get to the center. In this case the center is the root cause while the outer layer of the onion is apparent problem. The Five Why works best on front line problems that have a single root cause. To utilize the Five Why’s, we must first define the problem and write it down. Next, we ask the question “Why” and write down the immediate answer or cause. We repeat this procedure until we reach the root cause of the problem- this may be more or fewer than five times. Let’s take an example from the a book on Healthcare quality called the Nun and the Bureaucrat. In a particular hospital room, there was a problem with a medication cart. The medication cart was stopped working. Let’s following the 5 Whys. Why? Because the batteries powering it are dead. Why? Because the outlet for the charger is not working. Why? Because the outlet got wet. Why? Because water sprays on it from the sink nearby. Why? Because the water pressure is too high. This illustrates the idea that the immediate cause must be the answer to each “Why.” We can test this by going in the other direction. The water pressure is too high. Therefore, water sprays on the outlet. Therefore, the outlet gets wet. Therefore, the outlet for the battery charger stops working. Therefore, the batteries are dead. Therefore, the medication cart does not work. The Five Why’s is a basic but important element of operational excellence; it is just one small part of a robust problem solving process.
