Extracapsular Parotidectomy and Superficial Parotidectomy are both surgical techniques used to remove tumors (usually benign) from the parotid gland, the largest of the salivary glands. The distinction mainly lies in surgical approach and anatomical dissection, especially regarding the facial nerve. ⸻ ✅ Superficial Parotidectomy • Definition: Removal of the entire superficial lobe of the parotid gland. • Facial Nerve Dissection: The main trunk and branches of the facial nerve are formally identified and dissected to protect them during removal of the gland. • Indication: Most commonly used for benign tumors confined to the superficial lobe (e.g., pleomorphic adenoma). • Advantages: Safe, thorough; lower recurrence rate of tumor. • Risks: Facial nerve weakness (temporary or permanent), Frey’s syndrome. ⸻ ✅ Extracapsular Parotidectomy (ECP) • Definition: Removal of a tumor in the parotid gland with a margin of normal tissue but without identifying or dissecting the main facial nerve trunk. • Facial Nerve Dissection: Not performed unless the tumor is closely related to a nerve branch. • Indication: Small, well-encapsulated, benign tumors, usually located in the superficial lobe and away from major facial nerve branches. • Advantages: Less invasive, shorter operative time, potentially fewer nerve complications. • Risks: Higher chance of inadvertent nerve injury if anatomy is not clear; higher recurrence risk if margins are inadequate. #ear #ent #science #anatomy #biology #nose #nosebleed #otolaryngology #throathealth #tonsils
