The HPMC shell with Viscoat for Capsulorrhexis
This is a variation of the soft shell technique described by Dr Arshinoff where HPMC 2% is used primarily to coat the endothelium and a small blob of Viscoat ( a combination of chondroitin so4 + Sodium Hyaluronate ) is placed in the centre to tamponade and flatten the anterior chamber. They two VES have a clear visible interface. Sodium hyaluronate has a tendency to leave the eye en masse whereas dispersive VES are less prone to do this. This combination helps you to preserve the more expensive Viscoat for more number of cases, and is more economical than the soft shell combo. I use this for intumescent cataracts as well as to size the rhexis
