In intumescent cataracts using a high molecular weight heavy OVD to flatten the central portion of the lens and perform an initial mini rhexis of 2.o - 3.0mm, applying decompression and once the intumescence is successfully handled, to further enlarge the now quiescent capsule to the desired size of CCC with an Utrata or similar forceps .. is my go to technique and has worked more often than. This video shows you how to go about performing a double rhexis. Nothing ground breaking... yet good to know
