The surgeon started the case on small pupil, after having broken the posteior sinechia with a spatula, without needing to use any device for further enlargement of the pupil. But, at a certain point, an assimetric deepening of the anteior chamber led to suspicion of posterior capsule rupture and the sugeon decided to enlarge the pupil with iris hooks, in order to see exaclty what was going on behind the iris. Fortunately, the posterior capsule and the zonules were intact, the hooks were then moved on the anterior capsulorhexis, to stabilize the bag and the surgery continued uneventful. Foldable preloaded Zeiss Lucia has been safely implanted in the bag, then the hooks have been removed form the capsulotomy and iris.
