Ask A Pelvic Pain Specialist | What is Diastasis Recti?

Dr. Obzejta, a former pelvic pain specialist at PRM in Los Angeles, explains what diastasis recti is, who can experience the condition, and exercises that can help. At Pelvic Rehabilitation Medicine™, we believe that the millions of women and men who suffer from pelvic pain should have access to care that provides relief. We don’t believe “a glass of wine will fix it” or that “it’s all in your head”. We will never abandon you – and from leadership to the front office to every pelvic pain specialist you meet, so much of our team has been in your shoes and believes the pelvic pain patient deserves better. This belief is what unifies us all at PRM. Since 2017, PRM has provided an innovative solution to a health crisis that affects 15% of women and 10% of men. Persistent pelvic pain is often overlooked, untreated, and misguided. At PRM, our mission is to decrease the time patients are suffering from pelvic pain symptoms. We offer a proprietary, simple, office-based procedure to treat the symptoms of chronic pelvic pain. The PRM Protocol™ consists of a series of pelvic nerve and muscle treatments to directly target inflammation in the pelvis and nerve pain. Depending on the condition causing your pelvic pain, other modalities may be needed in addition to our treatment. Today we're talking about diastasis recti. So what exactly is it? Well, diastasis recti is a separation of the two halves of the abdominal muscles. We can see in males, females, even sometimes infants. However, most of the time we're going to see in our pregnant and postpartum patients. That's because with pregnancy and a growing baby, those muscles. of the abdomen are stretched, right? The good news is that connective tissue that holds the two halves of the abdominal muscles together is super stretchy, very resilient. So most of the time it's going to be able to go back to its original state. However, sometimes it needs a little extra help. And that's where physical therapy comes in. So diastasis recti is diagnosed with a simple physical exam. No advanced imaging is needed. Patients that suffer from diastasis recti can experience a jelly-like sensation beneath their belly button, even a bulging or coning. Sometimes it can even lead to pelvic floor dysfunction, urinary incontinence, low back pain. So definitely things to be on the lookout for. But like I mentioned, that connective tissue is super resilient and with, you know, proper help can go back to its normal state. However, it's super important to be under the care of a trained physical therapist as well as your physician. So you're not hurting yourself. There are exercises that are actually going to make it worse. So a lot of patients think, Hey, you know, if I just work on my core and do crunches and planks, it's going to get better. Those are no nos. Those are going to make it worse because we're increasing that abdominal pressure, right? So, we want to avoid V ups, traditional crunches, and planks. And exercises we do want to work on are diaphragmatic breathing, pelvic tilts, one legged, heel slides, as well as single legged heel drops.
