Along with this, we will discuss the pathogenesis of radicular cyst in 3 phases. We see how the radicular cyst is initiated, formed, and then expanded to resorb the surrounding bone. Clinical Features of Radicular Cyst and Radiographical features are than discussed which will guide the students to diagnose the radicular cyst pathology. When examined under the microscope we will find many histological features related to the radicular cyst, according to the 3 components of the cyst. Then we move on to the treatment of Radicular Cyst, conservative or surgical approach depending on the size of the cystic lesion. ▬ 📌 Radicular Cyst Radicular Cyst is an inflammatory odontogenic cyst of the jaws which is most commonly seen along with necrotic pulp of the tooth following inflammatory reaction due to physical, bacterial or chemical injury. So, this dental cyst has three types. 👉 Types of Radicular Cysts 🔴 Periapical Radicular Cyst 🔴 Lateral Radicular Cyst 🔴 Residual Cyst 👉 Pathogenesis of Radicular Cyst Next, comes the radicular cyst pathology. Pathogenesis of Radicular Cyst occurs in 3 stages: ⚫️ Initiation: Bacteria from the carious lesion enters the root canal system and eventually ends up in the periapical area, a granuloma is formed which causes a release of Lymphokines by the macrophages. ⚫️ Cyst Formation: Epithelial rest cells of Malassez present in the periapical region proliferate to form a mass, the central cells die of liquefactive necrosis and a cyst is formed. ⚫️ Cyst Expansion: Cyst becomes bigger by the action of osmosis. 👉 Clinical Features of Radicular Cyst Talking about the dental cyst symptoms, Radicular cyst is usually asymptomatic. ✔️ Carious Lesion of examination, there may be swelling, sensitivity or tooth mobility. ✔️ May even present as a bluish mass as the bone erodes. ✔️ And eggshell crackling sound on palpation may be heard. 👉 Radiographical Features of Radicular Cyst Radiographical Features of Radicular Cyst, includes: 🟢 Radiolucent lesion according to the type of cyst. 🟢 Loss of Lamina Dura 🟢 Root Resorption 👉 Histopathological Features of Radicular Cyst Histopathological Features of Radicular Cyst, includes: 🔵 Epithelial Lining is of Stratified Squamous Epithelium; Rushton bodies and mucous cells may also be seen. 🔵 Capsule is porous and comprises of collagenous fibrous Connective tissue, Inflammatory cells and Cholesterol clefts are also present. 🔵 Lumen consists of Eosinophilic material 👉 Treatment of Radicular Cyst Treatment of Radicular Cyst is done according to the size of the cyst: ✔️ If it is less than 2mm than a conservative approach is used, and Root canal treatment is done. ✔️ More than 2mm is an indication for a surgical approach, enucleation or marsupialization may be done.
