Hi, it's Dr. Nassif. I'm answering some blog questions sent to us on our YouTube channel. This one is scar tissue removal and Columella retraction or reduction possible. Let's talk about these two different things. You got to build up a scar tissue but we try to prevent scar tissue from being build up during rhinoplasty or right after the patient has a rhinoplasty. We'll inject something called 5-Fluorouracil in combination with a little bit of Kenalog, which is a steroid into the area where we may develop scar. If we do develop scar, we actually will inject that down also. Maybe a little bit stronger Kenalog. This can be done if it's straight 5-Fluorouracil with just a little bit of Kenalog every week even. If it has a little bit more Kenalog steroid, every four to six weeks. This is something that we do all the time, especially thick nasal skin and poor healing type of individuals. They get thick skin. Yes, we have to always worry about that. Columella Reduction means that this thing right here in the middle is hanging, that's usually not a scar tissue. Usually it's from some of the cartilage of a long septum or scar tissue or something we usually can just directly either excise in that tissue or just tuck it up. We call it tongue-in-groove suture. The answer is yes you can actually shave down or soften the whole Columella that's hanging.
