Vitrectomy for Submacular haemorrhage | Large submacular haemorrhages can be a huge challenge to manage. Various options such as gas and antivegfs and positioning are done often with the addition of TPA . In this case i prefer to evacuate the haemorrhage using a posterior retinectomy. i use mild diathermy to prepare the smiley shaped incision . I reduce the cutter speed to just 100 cuts a minute and it works like a well controlled scissor in this situation.Once the retinectomy is made i gently insinuate the cutter port into the opening and gently engage the thick haemorrhagic clot by using high vacuum.. The whole haemorrhagic clotted complex gets engaged and i gently deliver it out in toto. After this i use air fluid exchange, gentle endolaser to the site and gas injection followed by postoperative positioning.
