Unfortunately, treating these areas can be challenging, as retained secretions make it difficult for washes and medications to reach the sump effectively. For cases where maxillary sinus pneumatization extends below the floor of the nose, treatment requires a procedure called medial maxillectomy. However, when this extension is severe, treatment can become particularly complex and demanding. Sphenoid sinus sumps present similar issues but are more common in patients with well-pneumatized sphenoid sinuses. Here, sump formation often results from incomplete surgery where the floor of the sinus isn’t fully visible, leading to retained secretions and localized inflammation. To address a sphenoid sinus sump, surgeons often use a technique called the sphenoid sinus Lothrop procedure. This procedure aims to lower the front face of the sphenoid sinus to allow better visualization and access to the sump area, ultimately enabling more effective drainage.
