Bilateral abductor palsy is one of a very rare condition wherein the vocal cords gets paralyzed on both sides. Most of the times this is neurological and being neurological, it tend to resolve also gradually but comes back to the maximum unless there is a structural damage to the nerve where the nerve gets damaged due to a tumor or during a surgery of the nerve fibers are cut on both sides, then the vocal cords come close to the midline, this causing a operon to have a breathing difficulty. The botulinum toxin is quite effective when injected correctly and in the correct dose and the effectiveness lasts anywhere between 3 months and couple of years depending on the person’s body and the effects can be seen as soon as around 72 hours after injection. You can see the widening of the gap between the vocal cords visually and the person’s breathing improves significantly. The stridor or the noisy breathing also reduces significantly. A lot of people can come out of this tracheostomy if there has been any tracheostomy done. The only effect with the botulinum is that it is a little technically challenging procedure wherein the injection has to be done in the proper site and the proper dosage by a trained person. If that doesn’t happen, the whole issue becomes a little complicated. A lot of times we have seen that the toxin is digested faster and the person comes back with the recurrence of symptoms sooner. There are people who are getting injections done almost every year two to three times, it is quite safe to repeat it as there is no actually permanent damage done to the muscles and nerve fibers done. So it can be repeated as often to keep the person symptom free. So by chance the neurological symptoms improves and the palsy recovers, the person comes out, wherein we have already avoided destructive surgery in the larynx.
